To initiate a survey you must have received a form containing a Pass Code and completion instructions. The form is supplied by your Academy. If the Pass Code you have been given does not allow you to start the survey, contact your Academy to receive a new code.

* The Survey Identifier is used to identify your responses. You may use up to 20 characters or numbers. Be sure to record this code in case you wish to review, update or continue entering your survey responses at a later date. NOTE: Since the identifier may appear on individualized survey results, you may wish to use a code that only you will recognize.
